Submit it as best you can with the 5 discs.... mention the fact in the approver comments (the text box) that SLUS-01254 is also included (or just paste this list you've made here). I'll escalate it on the approver side if I see it... it'll probably be a matter of increasing the max number values (5 seemed like a good size when we made it.... 95% of the PS2 games are a single disc)
