To access the fourth difficulty level, Nightmare, start the game up, enter whichever difficulty level suits you then go straight towards the fourth episode.
Right after jumping into the pool of water, immediately reverse so that you're right under the ledge from which you stepped into the pool. Stay there for a bit, then you should have dropped onto a bridge above the ground. Follow the bridge
to the door on your left, enter and voila! Get ready to be rocked!
