Dan Barrows, a character appearing in one of the game's climatic fright scenes, returns in the sequel as the main antagonist. His physical form is greatly changed in the sequel. The PlayStation release of the original Clocktower attempts to explain this transformation in a cutscene.

The game's heroine, Jennifer Simpson, apart from sharing her first name also bears some physical resemblance to actress Jennifer Connelly. Connelly starred in the 1985 horror movie Creepers/Phenomena which is said to have provided several horror motifs used in Clocktower.

While only released in Japan, the SNES version has since been translated into English by fans.
