Fist of the North Star: Ken's Rage 2 is the sequel to Fist of the North Star: Ken's Rage and once again based on the original manga. The story follows Kenshiro, a master of the assassination art Hokuto Shinken, as he journeys through the nuclear wasteland that is now the world, defending the helpless, beating up enemies and defeating bosses. The plot is told through comic-book like panels with narration. Parts of the story from the first game are present again, especially those at the end of the first title, but expanded.

The game features a single campaign with multiple playable characters. Battles are typically conducted by being attacked by large waves of enemies. Kenshiro uses his various combos, special, and signature moves to defeat the enemy waves. The game features a Legend Mode and a Dream Mode. In Legend Mode, players go through a campaign with several playable characters. In Dream Mode, players can choose from the variety of characters available to them and play traditional style capture maps. Through Dream Mode, players level up their characters and gain new abilities, weapons, and upgrades along with several customization options.
