Honest Hearts does it’s best to get your head as involved in the narrative as possible to add gravitas to the choices you make while in it. And it pretty much forces you into making those choices. It asks you to inspect your idea of God, fate, and second chances. It puts you in the role of religious dispute arbitration consultant, and expects you to make a well thought out decision that will impact these people for the rest of their lives. Or at the very least to briefly consider the possibilities long enough to have a new thought in your head about the very subject. It’s not everyday that as a videogame character you embody the proof of God’s grace, and God’s existence for a man falling short, but in the same stroke become the reason that a pillar of faith and virtue has his belief shaken to his core. For this alone it is an experience that should be mandatory.

Honest Hearts is one of the better DLC packs Bethesda has released yet, and it comes down to the combination of an interesting landscape and original story along with unique RPG choices, then layering the signature Fallout brand (at least, since Bethesda took over) of slightly off-kilter, but still intensely entertaining FPS combat on top. If you enjoyed New Vegas enough to get at least most of the way through it, then this is surely a worthy addition to your DLC library. If the next two New Vegas packs as good as Honest Hearts wound up, then we'll be in for a treat this summer.

When considering the $10 or 800 Microsoft point purchase, ask yourself these questions: Do I like Fallout, but need a new place to explore? Do I want a higher level cap? Do I want to know more about the Burned Man? If the answer to any of these is “yes”, then do yourself a favor and download Honest Hearts. It doesn’t change much of the Fallout formula, which is a good thing. The narrative could be stronger and there’s a lack of variety in terms of creatures occupying the park, but these are minor complaints about an otherwise enjoyable time.

GameSpot (May 27, 2011)
This being a Fallout game, it's no surprise that you could run into a number of bugs. These include some enemies you cannot damage or target in the Vault-Tec Assisted Targeting System because they are standing in water; enemies hovering in midair or clipping into rocks; and non-player characters running around in circles as if their loincloths are on fire. Don't let such typical bugs dissuade you from considering Fallout: New Vegas - Honest Hearts, however. The story and related quests don't surprise, but this add-on gives you the opportunity to once again influence those you meet and accomplish your goals as you see fit. And, you do those things in a desert environment harboring enough creatures and caves to make it worth exploring.
Certain quests lack invention and the cast feels small, but otherwise another fine excuse to immerse yourself in the Fallout: New Vegas universe.

Honest Hearts has a wonderful setting with some fun things to do in it, but “some” just doesn’t cut it. The majority of the DLC is utterly devoid of surprises, treats or interesting characters and places, and you’ll soon start getting bored. It commits the worst sin an open-world RPG can do: it makes the player not want to explore. Stick to the Mojave: ain’t nothing here but rocks and broken dreams.

Gamekult (May 20, 2011)
GameSpy (May 23, 2011)
Strong concept, weak execution. Add in the dated look of the game, and numerous bugs and glitches new and old, and Honest Hearts isn't worth the $10.
GameStar (Germany) (Jun, 2011)
