From an anonymous Red Storm Entertainment engineer (as posted on fastclan.com):
"Way back when, Rainbow Six was going to be a third-person game (which is also the reason why auto-targeting was implemented). We even had a real cool second targeting window that had first-person in it. Then we allowed you to switch so your main window was first-person and the small window was third-person. Then we came to our senses, got rid of the small targeting window, and went completely first-person. Third-person was left in because some people still liked the view, and it was useful for observing."
