BattleBlock Theater is a 2D platform game. A group of friends aboard the ship S.S. Friendship become shipwrecked on a mysterious island after a heavy storm. All of the friends have been imprisoned, except for Hatty Hattington who has become the leader of the island's feline population. The levels of the game are presented as challenges set in a theater show for the cats' amusement. The story is furthered through cut-scenes in stick puppet style and through a narrator.

The player's character can move around, jump and attack. It is also possible to climb pipes, do wall jumps and double jumps. While combat with punches, kicks, weapons and special attacks can dispose of enemies, the focus is largely on the platforming. The game consists of eight worlds each with their own theme and split up into different stages. As the title implies, every part of the environment is built up out of blocks. These have different properties, causing them to disappear, become slippery with ice, and there are also conveyor belts, exploding platforms, portals, catapults, elevators etc. There are also hazards such as lasers, saw blades, spikes, traps and enemies. The gameplay combines Super Meat Boy challenging navigation with little room for error with environments that contain small puzzle elements at a slower pace. Puzzles often involve pushing blocks to influence the environment. Levels have checkpoints where the character respawns after dying.

300 new characters (prisoner friends) can be unlocked by collecting gems. Each character has a slight variation in movement and abilities. Three gems are required to complete a level, but there are always many more to be found. Each level also has a hidden yarn ball used as a currency to bribe guards for improved weapons.

Next to the main campaign there are 24 bonus stages (three per world). The player is also rewarded with a medal for a fast time (as well as an additional gem) and can locate hidden collectibles. The game includes a level editor to create and edit levels, and share them afterwards. Cooperative and competitive multiplayer is supported both offline and online. There are various game modes with King of the Hill or involving basketball.
