The entires for Entombed (1994) and Entombed Enhanced (1997) are totally mixed. The screen shots posted on Entombed Enhanced are from the Win 3.x version of Entombed from 1994.

That said, I am having difficulty finding screenshots that show exactly what the enhanced version looks like.

I can only verify the 1994 version and how it looks due to the fact that I own that verison. I actually have 2 versions of the 1994 release, v1.5 and 2.1. 1.5 requires the user to register their name and it brands the main menu with it, while 2.1 no longer requires this and updated the installer from a dos installer for win 3.1 to an actual win 3.1 installer.

edit: I was able to find the intro to the enhanced version on youtube: https://www.youtube.com/watch?v=JYju42J4e64

However it is unclear what systems it was released for.

edit: Evidence that the 1997 enhanced release was never released for windows 3.1 http://adventures-index12.blogspot.com/2010/04/entombed-enhanced.html

So basically, there is a 1994 win 3.1 release and a 1997 win 95 release. However the Win 3.1 release is called Entombed and comes from a different developer and publisher than the 1997 release which is titled Entombed Enhanced which was for Win 95.

So the decision is, so we combine them into one entry or does each release get it's own entry. If each release gets it's own entry, then this one should be renamed Entombed Enhanced. if they are combined, then the title on this one is fine but the box art for the first release needs to be added here and the other entry needs to be deleted.

If you download and play the demo, it calls itself shareware and contains the first chapter. "You can also order CD version" means it had a floppy disk edition.

Yes, however the official title "Entombed Enhanced" only applies to the 1997 release by Blue H2O. While technically the CD version of the game was "enhanced" in the sense that it had music and voice work, it was not actually titled any differently.

All versions of the 1994 game (cd or floppy) are simple titled Entombed. The CD version had a subtitle: http://i.imgur.com/mQ5mI9g.jpg

For some reason or another that subtitle has led some to assume that becuase it says "the ultimate surrealist adventure", that the cd version is Entombed Ultimate. Its not. It's just Entombed. All the title screens across the various versions simply say Entombed. The only difference is the version number. CD v1.5 also displyas the registered name of the user. The last nown version, CD v2.1 removed this and went back to just displaying Entombed.

The enhanced version was released 3 years later by a completely different publisher and developer. The box and the title screen of this version plainly say, Entombed Enhanced.

The screen shots currently on the entry for Entombed Enhanced are wrong and should be on the 1994 release. Entombed Enhanced was not compatible with Windows 3.1. It was Win 95 only.
