Beast Busters: Second Nightmare is an on-rails lightgun shooter for one or two players. It is the sequel to Beast Busters and runs on Hyper Neogeo 64 hardware featuring 3D modelled graphics.

The story from the intro: 2009 A.D the floating city of North Side Bay. "New Port Town". Within the International Specialized Pathologies Hospital located in this port town, there has been an outbreak of a horrible disease that turns people into hideous monsters. The U.N., which has been carefully monitoring the situation, dispatches three marine commandos to take care of the situation...

Players fight their way through 5 stages set in and around the hospital, fighting zombies, hounds, big insects and bats. Most enemies are zombies, some of which carry weapons like axes and guns. Players can dismember zombies by shooting off their head, limbs and even the entire torso causes big blood spurts and an explosion of guts. At the end of each stage there is a boss fight with giant monster, such as tentacled monsters in a bus and a large zombie with a chainsaw for an arm.

The guns have recoil effects and build in speakers for amplified gun sounds. Players can pull the pump back to reload or press the button on the side to shoot grenades. There are three types of grenades denoted with a number 1, 2, or 3. Grenade 1 throws a normal grenade into the screen, firing grenade 2 cause big on-screen machine guns to mow everything down, using grenade 3 fires a death ray from a satellite from outer space.
