The third I-Play-developed mobile game in the Metal Slug series continues the fast-paced 2D side-scrolling shooting action with a new main character: sergeant Allen O'Neil. He needs to protect his troops and civilization against an alien invasion through 13 missions set in 5 different environments. There are 3 difficulty settings, 3 level bosses, a range of different weaponry and backgrounds with destructible scenery and flying debris.

As in the previous game in the series, players can move left and right, or jump to reach higher areas. Using the keys, players can shoot in 8 different directions. Power-ups are provided by freeing humans or by shooting certain enemies or structures. Most weapon upgrades, such as the shotgun, flamethrower or laser gun, only come with a limited supply of ammo, while the base weapon carries an endless amount. A secondary button is used to throw grenades and in close combat a knife is used automatically. Certain weapon-powered vehicles such as a tank can be driven during the levels.
