Oshioki Kirai! 2 is a humorous arcade-style action game. The player controls a permanently running character who must escape from a giant human attempting to kill him. The game is divided into stages taking place in the same area but featuring different opponents. Each opponent is presented as an animated digitized image of a gigantic person towering over the stage while the protagonist keeps running in a pseudo-3D motion away from the depth of the screen. The player must move the hero left and right in order to avoid attacks. If the player character is hit three times he dies and the game is over. Surviving for a certain while eventually leads to the opponent's demise and the end of the stage.

The opponents have comical appearance and try to kill the protagonist with various household items, mostly connected to food. For example, one of the foes attempts to pierce the hero with chopsticks, while another cleans his nose and throws gigantic dirty napkins on him. Digitized photos of various related scenes announce new rounds in the game.
