Lethal League is a 2D fighting game based on the prototype browser game of the same name where characters use a single projectile as the main means of attack. Contrary to other fighting games, the characters cannot damage each other directly. Instead they have to hit a ball with a weapon to speed it up or to change its trajectory, causing it to hit the other character. There is an amount of strategy to it as the ball bounces around the screen and players can dodge or jump over it. A single hit also immediately ends the round. The environment consists of a single, non-scrolling screen. It is possible to jump and hit the ball mid-air. The direction of the ball can be changed prior to hitting, changing the direction from horizontal to diagonal for instance.

With every hit the speed of the ball increases with one point, shown through a boombox at the bottom of the screen. By performing a charged attack or a smash however the speed is instantly doubled. With increased speed the colours in the environment change and the screen starts to shake. New to this version of the game is that the ball changes to the character's colour when hit. That way it is clear who struck it and your own character cannot be damaged by it unlike in the original game. It is possible to hit your own ball multiple times in succession however. Also new is a bunt move that removes the colour if you struck it and removes all speed. It can be performed multiple times, but by following it up with a charged attack the speed from before is restored entirely. After four hits a character's bar is filled up and a special move can be performed. As soon as a character is hit, a point is awarded and the positions are restarted. The first player to reach a certain amount of points or to have the most points before the time limit runs out, wins the game. There are now different stages instead of just one in the original game.

The original game's character Raptor is supplemented with four additional ones: Candyman, Latch, Sonata and Switch. They each carry a different weapon, such as a baseball bat, a cane or a skateboard. They also differ in jumping methods, such as a double jump, a high jump, a wall jump or a wall ride. The characters can be moved around freely and can perform a short or a high jump, and pressing down makes them fall faster.

The game modes include a training mode, ten challenges, a versus mode and an online multiplayer mode. The latter two support up to four players simultaneously and the online portion uses the GGPO networking library. While playing against the computer there are seven levels of AI difficulty.
