Twisty Hollow is a puzzle game with similarities to time management games. The game is played on a round board that consists of three rings that each can be rotated with swipes. The inner ring contains workers, the middle ring tools and the outer ring materials. Outside the board are customers asking for specific product that the player has to create. This is done by spinning the rings so that a line of matching worker, tool and material is created. For example to make bacon, a butcher, a knife and a pig has to be lined up. By tapping on the worker production starts and when completed the player spins the outer ring until the product reaches the customer. The customer will then leave some coins that the player taps on to collect. The player gets a star rating based on how much money is made and to advance to the next level at least one star has to be earned.
After a few levels a time limit is added and customers will become angry if they get to wait too long. When they leave they might start a fire which has to be extinguished by lining it up with a fireman and a hose. Some products has to be created in multiple steps. For instance, to make a hamburger a steak first has to be made by matching, a butcher, a knife and a cow and then the steak has to be combined with the knife and a chef. As progress is made new characters are introduced as customers. Some levels involve animals and the player has to figure out which product they want. For example cats want milk or fish while dogs want meat. There is also a troublesome bear that can pop up and steal goods from the outer ring and he can also at times use his claws to stop it from spinning. There are also some special levels where the player has to hit robot moles. This is done by lining up the mole with a miner and a pick axe or with a farmer and a spade.
