Throwing a crushed can in the restroom. You can get the can in the orange wastebasket or in either of the ones on the sides of the sink.

The achievements are shown as items in the vending machine.

I threw a grenade and hit my co-worker.

I threw a grenade into the fish tank on the right.

I threw a grenade at the crates on the left in the warehouse and revealed a hidden wastebasket.

And I got that achievement.
