Reigns: Three Kingdoms is the fifth game in the Reigns series and the first historically-based installment, as well as the first set in Asia. As per the title, it is set in China during the Three Kingdoms era, when the region was split between the dynastic states of Cao Wei, Shu Han, and Eastern Wu. The player occupies a dual role: they are both an aspiring ruler of China who can raise an army to take over various regions, and a tester playing a simulation of such. As with other Reigns titles, death comes often, and every time the player dies they appear back in the present day, with a coordinator explaining the latest developments in the modern day plot. The player is then sent back into the simulation. Every time the player dies in the simulation they come back as a relative of their previous avatar, usually a distant cousin but also possibly a wife, husband or child.

Decisions are made by swiping left or right on each card; each decision affects the four traits displayed at the top of the screen: finances, public approval, military approval, and spiritual health. Bottoming out or maxing out the scores on any of these results in the current avatar's death, sending the player back to the present day to be reincarnated as a new avatar. Over the course of each life the player seeks to expand their avatar's influence across China, while also uncovering the truth behind the simulation.

Reigns: Three Kingdoms introduces some new features to the series; in order to "conquer" a region or otherwise settle disputes the player will be drawn into a card battle featuring characters they've met over the course of play. Each card has attack and health stats, and some have special abilities that will be activated in the fight when certain conditions are met. The cards are displayed in a wheel at the bottom of the screen, and the player turns the wheel and swipes the card to use it. The player has as many actions (attacking or turning the wheel) as indicated by the brass coins displayed on the screen. The opponent will then take their turn. When a card's health has been knocked down to zero, the card flips over and requires at least one action to replace in a future turn. The goal of each battle is to attack the provisions in the middle of each ring; the provisions are protected by character cards, so a player must knock out some of their opponent's cards before they can reach it. A battle ends once the provisions have been zeroed out. The game also has an online multiplayer option that lets players battle each other.

The game is a Netflix exclusive, only available to players with a Netflix account who login with such.
