Millie tells the story of a millipede who dreams about flying. The player must take Millie to the Aviation School by running through several mazes. The game plays like a crossing between Snake and Pac-Man. Each stage consists in a maze with one hole as exit that only opens when a certain number of pellets has been eaten by Millie. The more pellets Millie eats, the longer it becomes, making it difficult to run through the maze without hitting its own body. If that happens, the player has to try that level again. Besides pellets, Millie also collect power-ups: scissors will cut its tail, making Millie shorter; clocks will turn back time, undoing mistakes; beacon lights allow Millie to pass through one way streets; ice-skates allow Millie to run over ice; hammers break weakened walls and portals teleport Millie from one point of the maze to another.

The game is divided in 3 episodes with 32 levels each. Every fourth level is a flying one. In flying levels the player controls Millie's plane racing other planes, racing against time or through checkpoint courses with obstacles. All levels have a 3 star meter. Perfect scores are achieved by collecting all pellets and 3 pairs of shoes scattered through each maze. The last pair of shoes only appears if all pellets were collected.

The game has also a store where players can buy power-ups with stars collected by completing levels.
