Dr. Joan Aiken had discovered a way to inject a special serum into the human brain, which will grant this human powerful psychic abilities, known as "talents". Since only little babies were able to absorb the serum properly, it was injected to twin brothers, Cain and Abel, in a scientific project called "Children of Tomorrow".
When they grew up, the children took different paths. Cain joined a special law enforcement group, while Abel walked away and stepped into the world of crime. Years later, a radical organization known as Eye of Ra emerges, with the only goal to sabotage Children of Tomorrow project - according to them for ethical reasons. Cain's first mission is to infiltrate their headquarters and deal with their leader.
Sanity is an action game with puzzle-solving elements. There is also a role-playing element present in card-collecting and upgrading the talents. The game is heavy on combat, which is mostly ranged, involving Cain either shooting or using the psychic powers. The talents are classified into groups, such as Fire, Sun, Illusion, Science, Demonology, etc. In each group, there are many talents Cain can learn, which range from offensive and supporting combat spells to abilities needed to solve puzzles (such as levitating, pushing heavy objects, etc.). The game has fully 3D graphics with top-down perspective, similarly to Metal Gear Solid.

Despite the numerous hang-ups, crashes, and freezes users have reported, Monolith has stated it has no intention of releasing a patch for this game, suggesting that the majority of problems are the fault of graphics card incompatibilities rather than actual gameplay issues.
This, coupled with an unsuccessful marketing campaign, is probably one of the larger contributing factors which has led to new retail copies of this game being sold for $2.99 at game outlets such as EB and Software Etc.
